This patient had a prior closed rhinoplasty by a different surgeon about 2 years prior to consulting with Thomas Lamperti, MD. The patient was dissatified with several of the resulting changes that she was not looking for. The areas that she wished to change included her more upturned nasal tip which also appeared newly deprojected. This led to nostril flaring. Dr. Lamperti also noted pinching of the patient's middle third (internal nasal valve) area. After discussing treatment options the patient wished to proceed with an open revision rhinoplasty to correct these areas of concern. Dr. Lamperti placed bilateral extended spreader grafts to correct the middle third collapse. During the surgery Dr. Lamperti determined that during the patient's prior surgery the caudal (front edge) septum had been trimmed to create the increased tip rotation that the patient had experienced. To correct this, a caudal septal extension graft was carved using the patient's septal cartilage. This allowed Dr. Lamperti to re-lengthen (counter-rotate) the nasal tip a few millimeters by securing the nasal tip cartilages to the combined spreader graft/extension graft complex. While repositioning the tip Dr. Lamperti also restored the patient's tip projection. To treat the nostril flaring a Y-V alar base reduction was also performed. In the 2 year after photos, you can appreciate how the patient's nasal tip is less upturned (but still feminine) and the nostril flaring improved. The middle third of the nose also has improved support and pinching. Note how on right profile view the mid-bridge is now less sunken/more smooth.
